ABSTRACT

OBJECTIVE@#The prevalence and related factors of serum anti-HCV in different regions and hospitals have not been studied extensively in China. We used routine screening data to determine the prevalence of HCV antibody in hospital patients, evaluate the epidemic trend of hepatitis C and formulate screening strategies.@*METHODS@#Patient information and HCV antibody testing results were collected from January 2017 to December 2019 in 77 HCV sentinel hospitals in China. Univariate and multivariate logistic regression was used to determine the characteristics and associations.@*RESULTS@#HCV antibody prevalence rates were distinct among patients in different departments, with a range of 0.33%-6.93%. Patients who were admitted to the liver disease-related departments (a OR = 10.76; 95% CI, 10.27-11.28), Internal Medicine (a OR = 2.87; 95% CI, 2.75-3.00), and Department of Surgery (a OR = 1.95; 95% CI, 1.87-2.04), were more likely to be tested for HCV antibody positive. HCV antibody prevalence was associated with patients aged 45 years and older (a OR = 2.74; 95% CI, 2.69-2.80), testing in infetious disease hospitals (a OR = 2.33; 95% CI, 2.26-2.40) and secondary hospitals (a OR = 1.72; 95% CI, 1.69-1.75). Patients in sentinel hospitals of the Northeast (a OR = 12.75; 95% CI, 12.40-13.11), the Central (a OR = 1.65; 95% CI, 1.61-1.70), and the West (a OR = 1.78; 95% CI, 1.73-1.83) China had higher HCV prevalence than those who were in the Eastern coastal area.@*CONCLUSION@#Those who were over 45 years old and saw doctors for liver diseases, and invasive diagnosis and treatment should be referred to HCV antibody testing.

ABSTRACT

Objective To analyze the epidemic characteristics of influenza in children in Jinshan District of Shanghai,so as to provide scientific basis for prevention and control of influenza in children. Methods We analyzed the surveillance data as well as the results of virus isolation and identification in children with influenza-like illness (ILI) in Jinshan District of Shanghai from January 2010 to December 2019. Results There were two annual epidemic peaks in 2010, 2016, 2017 and 2018, one annual epidemic peak in winter and spring in 2011 and 2013, and two annual epidemic peaks in 2012, 2014, 2015 and 2019. A total of 3 859 ILI cases were collected in the past 10 years, and 848 positive virus strains were isolated, with a positive rate of 21.97%. Among them, 371 were seasonal H3N2, 163 were new H1N1, 314 were type B(among them, 226 were yamagata and 88 were victoria). The positive rates of influenza virus in different children groups were 0.88% in scattered children, 6.76% in kindergarten children, and 14.33% in students, respectively (P= 0.001). Conclusion From January 2010 to December 2019, seasonal H3N2 was the predominant virus strain in children influenza epidemic in Jinshan District of Shanghai. The influenza epidemic shows a strong seasonal feature with double peaks in winter/spring, summer or autumn. The positive rate of influenza virus in middle school students is the highest in different age groups, followed by kindergarten children.

ABSTRACT

ObjectiveTo analyze the effects of respiratory control measures before and after COVID-19 epidemic on influenza virus. MethodsThe percentage of influenza-like cases, the positive rate of influenza virus and the change of influenza outbreaks before and after the COVID-19 pandemic were compared and analyzed by selecting the data of influenza surveillance sentinel-points in Shanghai. ResultsThe percentage of influenza-like illness after the outbreak of COVID-19 in 2020 was significantly higher than that during the same period between 2017 and 2019. The positive rate of influenza virus detection in 2020 was significantly lower than the average rate of influenza virus detection from 2017 to 2019 with significant statistical difference (χ 2=2 359.07, P<0.001). The number of outbreaks in 2020 was significantly lower than that from 2017 to 2019. ConclusionDuring the respiratory season, personal protection and reduction of human aggregation can effectively reduce the infection of influenza and the incidence of influenza in the population.

ABSTRACT

Objective@#To investigate the epidemiologic characteristics of Norovirus among adult patients suffering from infectious diarrhea in Shanghai, so as to provide evidence for developing related strategies on prevention and control of the disease.@*Methods@#Diarrheal outpatients were monitored at the 'Intestinal clinic’ from 22 hospitals involved in the sentinel surveillance program in Shanghai. Information on demographic and epidemiologic features of the patients was collected while data and clinical, fecal specimens were collected and sent to the district CDC for Norovirus detection. Positive rates of Norovirus were also compared in various populations and seasons during 2013-2018. Multivariate logistic regression model was adopted to fit into the comparisons between non-Norovirus and Norovirus groups.@*Results@#19.28% of the 12 083 diarrheal cases were found to have carried the Norovirus, with GⅡgroup the most commonly identified genotype. Rates of detection was seen higher in males (20.78%) than in females (17.73%). 30-44 year-old were found having the highest positive rate (21.51%). The positive rates were found the highest (23.60%) in the year of 2015. All the above shown differences were statistically significant (P<0.05). Diarrheal patients affected with Norovirus would present watery stool (75.94%) and vomiting (35.84%). Data from the multivariate logistic analysis showed that factors as: being males, 30-44 years old, officials/clerks, in winter season and with histories of travelling etc., were related to higher risks on Norovirus infection.@*Conclusions@#Infectious diarrhea caused by Norovirus occurred all year round, with seasonal peaks seen in winter and spring, in Shanghai. Specific prevention and control measures should be taken on Norovirus- caused infectious diarrhea, according to the difference on age, gender and season.

ABSTRACT

OBJECTIVE:To introduce the research and application of sentinel hospital pharmacovigilance system,and provide technical reference for hospital pharmacovigilance working in China. METHODS:A sentinel hospital pharmacovigilance system based on hospital information system was established,the architecture and functionality were introduced and its effects were ana-lyzed. RESULTS:The sentinel hospital pharmacovigilance system formed by hospital business information management platform and data center. Its main functions included drug data arrangement,the auxiliary reporting of ADR,active monitoring,pharmaco-vigilance information inquiry,monitoring and warning and statistical analysis,which successfully achieved the rapid reporting and active monitoring of hospital ADR. The system had applied in 20 sentinel hospitals,and the ADR reporting quantity was obviously increased after applying the system. Compared with 2015,ADR reporting in a sample sentinel hospital was increased 120.6% since it used the system in early 2016. Besides,the system had improved the ADR reporting process,operation and input standard for the ADR reporter,shortened the reporting time and improved the efficiency of the reporting staff. CONCLUSIONS:The establish-ment and application of sentinel hospital pharmacovigilance system has greatly improved the hospital ADR monitoring management level,and it is of great significance to further strengthen the pharmaovigilance in China.

ABSTRACT

Objective To analyze the characteristics on injuries,using data from the Jiangsu sentinel hospitals,from 2006 to 2013,to develop a injury prevention strategy.Methods Data was collected from the ‘National Injury Monitoring Report Card' of six sentinel hospitals from two surveillance points from 2006 to 2013.Results Information from 448 778 cases was analyzed.The numbers of male cases outnumbered the females,with sex ratio as 1.72:1.The top five causes of injuries were as follows:falls (27.21%),traffic-related (26.23%),blunt (17.50%),knife/sharp instrument (13.87%) and animal injury (6.59%).Injuries mainly occurred in June,July and August,at 8 to 10 O'clock in the morning.The major places where injuries occurred were road/street (31.58%),home (23.47%),industries and construction sites (21.38%).Recreation activities (36.12%),driving vehicle (25.26%),and at work (24.78%) were the major activities when injuries took place.Most injury cases were unintentional(92.65%) and deliberately(violence/attack) (6.36%).Conclusion All the injuries showed an yearly increase,from 2006 to 2013,with main causes as falls and traffic-related ones.Injuries among teenagers and the elderly should be paid special attention to,with targeted interventions taken.
